RESUMO

Background: Natural water sources are considered as the major environmental exposure of fluoride, resulting in increased prevalence of enamel fluorosis. This type of natural exposure should be permanently monitored to avoid the interactions with other non-natural fluoride sources. We evaluated the prevalence of enamel fluorosis in Colombian schoolchildren and its relationship with fluoride-containing water ingestion exposure dose and urinary fluoride excretion. Material and Methods: We included 923 schoolchildren aged 7-12 years residing in eight municipalities in Colombia. Sampling of consumption water was performed in major aquifers used for daily supply. Samples were collected in 98 polyethylene containers and refrigerated until analysis. Water and urine fluoride concentrations were measured using the fluoride selective electrode method. Enamel fluorosis was evaluated using Thylstrup and Ferjerskov Index (TFI). Demographic and anthropometric characteristics were assessed. Besides, other exposures to non-natural fluoride were also evaluated. Logistic regression was applied for multiple analyses. Results: The median fluoride concentration in water and urine samples was 10.5 mg/L and 0.63 mg/L respectively, with the highest value found in Algarrobo-Magdalena, and the lowest value found in Manzanares-Caldas. The overall prevalence of enamel fluorosis was 86.1%, being more frequent the mild codes with TFI-1 to TFI-2. The highest prevalence was found in Margarita-Bolívar and Manzanares-Caldas, and the most severe codes (TFI-5 to TFI-9) were detected in Manzanares-Caldas. The multiple analysis revealed water ingestion exposure dose, urinary excretion, involuntary intake of toothpaste, amount of table salt consumption and sex as significant factors (p< 0.001). Conclusions: The fluoride ingestion exposure dose and its subsequent urinary excretion could be used as estimators of past fluoride exposure, explaining the current prevalence of enamel fluorosis in Colombian schoolchildren. Resumen Se caracterizaron especies de Candida recuperadas de la cavidad bucal de pacientes oncológicos en una institución prestadora de salud de Barranquilla, Colombia. La investigación tuvo un diseño de corte transversal, mediante un muestreo a conveniencia se seleccionaron 60 pacientes oncológicos con candidiasis oral. Las muestras obtenidas se cultivaron en agar Sabouraud cloranfenicol, CHROMagar® Candida y agar Sabouraud dextrosa. Los microorganismos aislados se identificaron y se estableció el perfil de sensibilidad a los antifúngicos. Se aplicó estadística descriptiva, prueba de X 2 y análisis bivariado utilizando el programa Statgraphics Centurión XVII, con razón de momios (OR) para probabilidad de ocurrencia. Se identificaron un total de 107 cepas de Candida correspondientes a 15 especies, distribuidas del siguiente modo: C. albicans 23%, C. glabrata 18%, C. tropicalis 13%, C. krusei 10%, C. intermedia y C. lipolytica con 1,5%. En 77% de los casos estuvieron implicadas especies diferentes de C. albicans. Se identificó la existencia de relación entre cáncer del sistema reproductor y C. guilliermondii (p = 0,0001, < 0,05; OR 17) y entre C. colliculosa y cánceres respiratorios (p = 0,0003 < 0,05; OR 19,5). El 99% de las especies de Candida identificadas fueron sensibles a los antifúngicos: fluconazol, voriconazol, caspofungina y micafungina. Solo una cepa de C. krusei fue resistente. Se concluye que existió diversidad de especies de Candida en la cavidad bucal de pacientes oncológicos, ya sea como colonizantes únicos o en combinación, de modo que más de una especie de Candida podría ser responsable de la infección micótica en la cavidad bucal de estos enfermos.

With regard to antifungal susceptibility, 99% of the identified Candida species were susceptible to the following antifungals: fluconazole, voriconazole, caspofungin, and micafungin. Only one strain of C. krusei was resistant. It is concluded that there was a diversity of Candida species, either single or mixed in cancer patients, which could determine that only one species is not responsible for fungal infection in the oral cavity.

Introducción: La anestesia general es considerada como una herramienta farmacológica que forma parte de las técnicas avanzadas en odontología para el manejo de la conducta de pacientes poco colaboradores o con alguna discapacidad física y/o mental, sirviendo como mecanismo de soporte en la ejecución de tratamientos odontológicos. Objetivo: Evaluar el uso de la anestesia general durante la atención odontológica a niños y personas con discapacidad. Métodos: Revisión sistemática de la literatura en la que se analizaron 17 estudios publicados entre el 2009 y el 2019, seleccionados de seis bases de datos (MedLine-PubMed, LILACS, SciELO, Clinical Key, IBECS y Cuiden). Para el estudio se tuvieron en cuenta las recomendaciones del Preferred Reporting Items for Systematic Reviews and Meta-Analyes (PRISMA) y se mantuvo el rigor ético y metodológico correspondiente. Resultados: Los resultados de la búsqueda permitieron caracterizar los estudios por año, país y abordaje metodológico. La evidencia muestra que el uso de la anestesia general como técnica odontológica es útil y eficaz para el manejo de conducta en pacientes pediátricos y discapacitados. Conclusiones: La anestesia general es una excelente alternativa como técnica odontológica para la resolución de problemas bucodentales en niños y personas con discapacidades físicas y/o mentales y en el mejoramiento de su calidad de vida(AU)


Introduction: General anesthesia is considered to be a pharmacological tool included among the advanced dental techniques used to manage uncooperative and physically and/or mentally disabled patients, as well as a support mechanism in the performance of dental treatments. Objective: Evaluate the use of general anesthesia for the dental care of children and people with disabilities. Methods: A systematic bibliographic review was conducted of 17 studies published from 2009 to 2019 in six databases: MedLine-PubMed, LILACS, SciELO, Clinical Key, IBECS and Cuiden. The study followed the recommendations contained in Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and maintained the corresponding ethical and methodological rigor. Results: Search results made it possible to characterize the studies by year, country and methodological approach. The evidence obtained shows that general anesthesia is useful and effective as a technique to control the behavior of children and people with disabilities during dental care. Conclusions: General anesthesia is an excellent alternative as a technique to solve the oral problems of children and people with physical and/or mental disabilities, thus improving their quality of life(AU)

RESUMEN Introducción: Algunos tipos de cáncer suelen ser tratados con fármacos quimioterapéuticos que afectan de forma indirecta la cavidad bucal, los cuales susceptibilizan al paciente a contraer infecciones bucales como candidiasis bucal. Se ha evidenciado que una condición oncológica facilita una infección bucal por Candida albicans, sin embargo, otras especies de Candida se les relaciona poco. Objetivo: Identificar especies de Candida spp. en cavidad bucal de pacientes sometidos a quimioterapia. Métodos: Estudio descriptivo de corte transversal. Se tomaron 60 muestras de igual número de pacientes por conveniencia de un Instituto Oncológico en Barranquilla, Colombia. Previo a un examen clínico de diagnóstico de candidiasis bucal. Se tomaron hisopados en las superficies infectadas para realizar un examen directo -tinción de Gram y cultivo en agar dextrosa Sabouraud con cloranfenicol incubadas a 37 ºC durante 48 h y posteriormente cultivadas en medio CHROMagar® Candida- para identificación de diferentes especies. Se aplicó estadística descriptiva y de correlación. Resultados. El 55 por ciento de los pacientes correspondió al sexo femenino, con edades mayores a 60 años. El cáncer de mama fue el más frecuente en el 30 por ciento. La quimioterapia aplicada a todos fue el 5-fluorouracilo combinado con ciclofosfamida. El tipo clínico de candidiasis atrófica en lengua fue más frecuente en el 37 por ciento, seguida de candidiasis pseudomembranosa en lengua en el 33 por ciento. El 90 por ciento de muestras presentó algún tipo de Candida, crecimiento de una única especie y cultivos mixtos con más de una especie de Candida. Se evidenció el 13,3 por ciento de C. albicans y otro 13,3 por ciento de Candida spp. no albicans. Ninguno de los diagnósticos oncológicos tuvo una asociación estadísticamente significativa con las especies de Candida. Conclusiones: Se evidencia que la especie de C. albicans en candidiasis bucal en pacientes con cáncer, fue la más frecuente, con predominio en mayor proporción cultivos mixtos de Candida no albicans (C. glabrata, C. tropicalis) poco relacionados con este tipo de enfermedad(AU)


ABSTRACT Introduction: Some types of cancer are often treated with chemotherapeutic drugs which indirectly affect the oral cavity, thus making the patient vulnerable to infections such as oral candidiasis. Oral infection by Candida albicans has been found to be fostered by oncological conditions, but other Candida species have not been sufficiently analyzed in such a context. Objective: Identify Candida spp. in the oral cavity of patients undergoing cancer chemotherapy. Methods: A descriptive cross-sectional study was conducted of 60 samples of an equal number of patients selected by convenience sampling at an Oncological Institute in Barranquilla, Colombia, following diagnostic clinical examination for oral candidiasis. Swab samples were taken from the infected surfaces to perform direct examination - Gram staining and Sabouraud dextrose agar culture with chloramphenicol incubated at 37ºC for 48 h and then cultured in CHROMagar® Candida medium - for identification of various species. Analysis was based on correlation and descriptive statistics. Results: Of the total patients evaluated 55 percent were female and aged over 60 years. Breast cancer prevailed with 30%. The chemotherapy applied in all cases was 5-fluorouracil combined with cyclophosphamide. Atrophic candidiasis of the tongue was the most common clinical type with 37 percent, followed by pseudomembranous candidiasis of the tongue with 33 percent. Of the samples examined, 90 percent contained some sort of Candida, growth of a single species and mixed cultures with more than one Candida species. C. albicans represented 13.3 percent and non-albicans spp. another 13.3 percent. None of the oncological diagnostic analyses had a statistically significant association to Candida species. Conclusions: The species C. albicans was the most commonly found in oral candidiasis among cancer patients, with a predominance of non-albicans Candida spp. (C. glabrata, C. tropicalis) mixed cultures, a fact not generally related to this condition(AU)

Chemotherapy and radiotherapy are aggressive treatments for cancer management. Both therapies make the stomatogatic system vulnerable to adverse effects on the oral mucosa and hard tissues. This may result in severe oral complications that can affect the quality of life of the oncologic patient. Consequently, oral diagnosis and interdisciplinary management by the stomatologist are critical for cancer treatment, regardless of its location. Objective. To determine the oral health status of cancer patients before, during and after antineoplastic treatment at a cancer institute in the city of Barranquilla, Colombia. Materials and Methods. A descriptive, longitudinal and prospective study of 131 cancer patients, was conducted. The study consisted of initial stomatological assessment of the antineoplastic therapy; classification according to the antineoplastic therapy given by the oncologist; a second stomatological assessment during treatment; and a final stomatological assessment or evaluation forty days after the end of therapy. Descriptive statistics, chi-square and MacNemar test were used to compare and identify variances at the different stages. Results. Female patients accounted for 69%, and breast cancer had 24% prevalence among the included subjects. At the initial stomatological assessment, high frequency lesions were identified, such as generalized biofilm-associated gingivitis in 69% of the cases, followed by oral candidiasis in 61%. The specific prevalence of lesions was 10.65%. In the second stomatological assessment, a greater frequency of periodontal abscesses was observed in 31%, and oral mucositis type II in 18%. The third clinical assessment showed significant changes in oral health status; an increase in the frequency of gingivitis was found in 9.9% (p<0.001); unlike before and during, there was an increment in dental caries of 26.73% (p<0.00000) at this last stage, root remains increased by 39.53% (p<0.00000), and finally, xerostomia increased by 45%. Oral candidiasis was the only lesion that showed improvement. Conclusion. An increase in the number of lesions was observed during and after antineoplastic treatment. The oral cavity is susceptible to antineoplastic treatments; gingivitis, candidiasis, xerostomia, and mucositis were observed, among others conditions.


La quimioterapia y la radioterapia son tratamientos agresivos para el manejo del cáncer, producen susceptibilidad en el sistema estomatogático causando efectos adversos en mucosa bucal y tejidos duros. Esto se traduce a complicaciones bucales agresivas, que afectan la calidad de vida del paciente oncologico, por lo que es fundamental el diagnostic bucal y manejo interdisciplinario que incluya el estomatologo en manejo del cáncer, indistintamente de su localizacion. Objetivo. Determinar el estado de salud bucal antes, durante y después del tratamiento antineoplásico en un instituto oncológico de la ciudad de Barranquilla. Materiales y Métodos. Estudio descriptivo prospectivo longitudinal, con una muestra de 131 pacientes con cáncer. Constó de: valoración estomatológica inicial a la terapia antineoplásica, clasificación según la terapia antineoplásica asignada por el oncólogo, una segunda valoración estomatológica durante los tratamientos, y finalmente una última valoración estomatológica cuarenta días de culminadas las terapias. Se empleó estadística descriptiva, chi cuadrado y prueba de MacNemar para comparar e identificar varianzas en las diferentes fases. Resultados. Un 69% eran del género femenino con frecuencia de cáncer de mama en un 24%. A la valoración estomatológica inicial se identificó alta frecuencia de lesiones como gingivitis asociada a biofilm generalizada en un 69%, seguida de candidiasis oral en un 61%. La prevalencia puntual de lesiones fue de 10,65%. En el segundo examen estomatológico se observó mayor frecuencia de abscesos periodontales en un 31% y mucositis oral tipo II en un 18%, entre otras. La tercera valoración clínica mostró cambios significativos en la salud bucal; se encontró un aumento de la frecuencia de gingivitis en un 9,9% (p<0,001) a diferencia del antes y el durante, igualmente para la caries dental se encontró aumentada en un 26,73% (p<0,00000), restos radiculares aumentó en un 39,53% (p<0,00000) y finalmente la xerostomía aumentando en un 45%, entre otras; la única lesión que mostró mejoria fue la candidiasis oral. Conclusión. Se observó un aumento de las lesiones, durante y después del tratamiento antineoplásico. La cavidad oral es susceptible a los tratamientos antineoplásicos, se relacionan con: gingivitis, candidiasis, xerostomía, mucositis entre otras.

ABSTRACT. Cancer control treatments are aimed at prolonging patient survival, but they also trigger inevitable and irreversible side effects, often infectious in nature, complicating the prognosis of the oncological disease. These include the pathogenic behavior of Candida, with Albicans being its most common species in the oral mucosa. Albicans is considered a commensal fungus that, as a result of the neutropenia resulting from antineoplastic therapies, behaves as an opportunistic entity causing candidiasis. This fungal infection has aggravating factors for the patient, such as xerostomia and hyposalivation, infections like dental caries, and other factors such as improper oral hygiene. Several consulted authors highlight the presence of Candida, its different species and oral candidiasis as a microorganism and classic opportunistic infection in immunosuppressed patients, and even more in those subjected to antineoplastic treatments. The aim of this review is to describe the pathogenic behavior of Candida in the oral mucosa of patients under antineoplastic treatments.


RESUMEN Los tratamientos para el control del cáncer están encaminados a prolongar la sobrevida del paciente, pero al mismo tiempo desencadenan efectos inevitables e irreversibles, así como infecciosos, los cuales complican el pronóstico de la enfermedad oncológica. Dentro de estos últimos se encuentra la conducta patógena de Candida, cuya especie más común en la mucosa oral es Albicans, considerada un hongo comensal que, a consecuencia de la neutropenia resultante de las terapias antineoplásicas, se comporta como oportunista y causa candidiasis. Esta infección fúngica tiene factores agravantes e inherentes al paciente, como la xerostomía y la hiposalivación, infecciones como la caries dental, y otros factores como la incorrecta higiene bucal. Diversos autores consultados resaltan la presencia de Candida, sus diferentes especies y la candidiasis bucal como microorganismo e infección clásica oportunista en pacientes inmunosuprimidos, y aún más en aquellos sometidos a tratamientos antineoplásicos. El objetivo de la presente revisión consiste en describir el comportamiento patógeno de Candida en la mucosa bucal de pacientes bajo tratamientos antineoplásicos.

BACKGROUND: Links between the demanding nature of studies in the health sciences, students' personality traits and psychological distress have been well-established. While considerable amount of work has been done in medicine, evidence from the dental education arena is sparse and data from Latin America are lacking. The authors conducted a large-scale investigation of psychological distress among dental students in Colombia and sought to determine its curriculum and student-level correlates. METHODS: The Spanish version of the Derogatis' Symptoms Checklist Revised (SCL-90-R) was administered to all students officially registered and attending classes or clinics in 17 dental schools in 4 geographic districts of Colombia between January and April 2012. Additional information was collected on participants' socio-demographic information and first career choice, as well as school's characteristics such as class size. The Global Severity Index (GSI) score, a measure of overall psychological distress, served as the primary analytical endpoint. Analyses relied on multilevel mixed-effects linear and log-binomial regression, accounting for study design and sample characteristics. RESULTS: A total of 5700 dental students completed the survey, a response rate of 67%. Pronounced gradients were noted in the association between socio-economic status and psychological distress, with students in higher strata reporting fewer problems. After adjustment for all important covariates, there was an evident pattern of increasing psychological distress corresponding to the transition from the didactic, to the preclinical and clinical phases of training, with few differences between male and female students. Independent of other factors, reliance on own funds for education and having dentistry as the first career choice were associated with lower psychological distress. CONCLUSIONS: Levels of psychological distress correlated with students' socio-economic and study-level characteristics. Above and beyond the influence of person-level factors, variations in levels of distress paralleled specific transitional stages of the 5-year dental curriculum, providing opportunities for targeted interventions.
